Replicate PEPFAR's localization model across all of USAID

Partially done

Printed page 263 9. Agency for International Development

“Over the four years of the Trump Administration, the multibillion-dollar program increased the amount of funding disbursed to local entities from about 25 percent to nearly 70 percent with positive overall results. This model should be replicated across all of USAID.”

What has happened

  1. Partially done

Extended beyond health. The PEPFAR transition strategy delivered to Congress on 2026-06-10 commits every partner country to a two-to-five-year structured transition to country-led ownership with declining U.S. budgets and "an effort to move from international implementing partners to direct government-led programming"; the locally-led development report of 2026-06-30 applies the same approach to development and humanitarian assistance. `partial` and not `done`: the Mandate's model was a rising share of funding disbursed to local entities, and the replicated mechanism routes money to recipient governments and to OCHA-administered pooled funds rather than to local implementers as such - the published local-partner share of the first OCHA contribution is 13 percent, not the 70 percent the Quote cites for PEPFAR. USAID, the organization the Quote said to replicate it across, no longer exists.

  2. Partially done

The America First Global Health Strategy extends PEPFAR's country-ownership logic across all U.S. health foreign assistance: multi-year bilateral agreements with recipient governments, co-investment and performance benchmarks, funding for 100 percent of frontline commodity purchases and frontline healthcare workers, and a shift of technical assistance "from supporting individual clinical sites to supporting governments in taking over key functions", explicitly leveraging the private sector and faith-based organizations. The strategy's own diagnosis is the Mandate's: only about 40 percent of PEPFAR's budget reaches service delivery, and Uganda alone had 57 implementing partners and more than 300 sub-recipients in FY2024.
